If you are looking for a copy of a Sacramental record, you must send us certain pieces of information either by regular mail, fax or email:
* A copy of photo ID
* Request in writing for particular certificate(s)
* Name of recipient of Sacrament and birth date
* Parents names
* Your phone number for clarification purposes
* How you would like to receive the certificate (by regular mail, pick up or mail directly to another parish)
